ONE OF THE LEADING PEDIATRIC SURGEONS OF RUSSIA VISITED THE DEPARTMENT OF PEDIATRIC SURGERY


On February 4-6, 2023, the Honored Doctor of the Russian Federation, Chief Physician of the Children’s Clinical Hospital of the Irkutsk Region, Corresponding Member of the Russian Academy of Sciences, Professor Yury Andreevich Kozlov visited the Tashkent Pediatric Medical Institute, departments of faculty and hospital pediatric surgery and performed high-tech minimally invasive operations.
And also Professor Kozlov Yu.A. held practical classes for students and masters on the topic of minimally invasive high-tech surgical treatment of birth defects (anomalies).
SCIENTIFIC AND PRACTICAL COOPERATION BY RUSSIAN SCIENTISTS
On March 10-12, 2023, employees of the Russian State Medical University. Pirogova d.m.s. Kholostova Victoria Valerovna, Ph.D. Yarustovsky Pavel Mikhailovich and Al-Mashat Namir Adnanovich held a master class. They performed operations on 3 patients, while performing the following surgical procedures.
1. Surgery to eliminate fistulas with the help of laparoscopic video assistance in recto-vestibular type of atresia
2. In the syndrome of the superior mesenteric artery, duodeno-jejunoanastomosis operation under laparoscopic video-assistance.
3. With the total form of Hirschsprung’s disease, the operation is reconstructive ileo-rectoanastomosis.
Scientific achievements of the Department of Faculty Pediatric Surgery in the period 2020-2022
PZ-2017083014 “Improving the treatment and rehabilitation of children with anal incontinence”
The results of rehabilitation of 279 patients who underwent perineal and abdominoperineal proctoplasty for malformations of the anorectal region in the Department of Elective Surgery of the TashPMI clinic were analyzed. Patients were studied in 2 groups. 1 – a group of 151 patients (2015-2017) for whom rehabilitation was performed incompletely or the stages of implementation were not observed, in some cases it was not carried out at all. In the 2nd group (2018-2020), rehabilitation measures were carried out with full control based on the developed rehabilitation protocol in 128 patients.
A survey was conducted on the Wexner scale in 42 patients with fecal incontinence of the 1st degree: of them, 19 patients in the 1st group had an average of 9.4 points and in 23 patients from the 2nd group, as a result of proper rehabilitation treatment, this indicator amounted to an average of 5.5 points. Out of 37 patients with grade 2 fecal incontinence, an average of 13.7 points was noted in 17 children from group 1 and 8.1 points in 20 patients from group 2. In 27 patients diagnosed with grade 3 fecal incontinence, an average of 15.3 points was recorded in 11 patients from the 1st group and 12.3 points in 16 patients from the 2nd group. The results obtained using sphincterometry were directly proportional to these data.
The rehabilitation protocol that we have developed will undoubtedly help to achieve good and satisfactory results if it is carried out correctly and in a timely manner, following the step-by-step recommendations for rehabilitation in the postoperative period for anorectal malformations. This allows sick children to quickly adapt to society and return to a full life.
PZ-2017090420 “Improvement of early diagnosis and method of surgical intervention of congenital hydronephrosis in children”
To conduct a retrospective analysis of the long-term results of surgical treatment of VH, we created two clinical groups. Group I consisted of 79 children who underwent vessel-preserving pyeloplasty according to the developed modification. Group II – 34 patients who underwent Anderson-Hines pyeloplasty. At the same time, we studied the course of early postoperative and long-term results of surgical treatment in both groups.
The results of clinical observations of the early postoperative period demonstrate that vessel-preserving pyeloplasty (group I) has a number of advantages compared to Anderson-Hines pyeloplasty (group II). In patients of group I, it is possible to accelerate the healing processes and restoration of peristaltic waves of the pelvis and ureter and to achieve earlier removal of drainage tubes compared to children of group II. At the same time, the average drainage time of the collecting system of the kidney in children of group I was 7.0 + 0.5 days, and in group II – 9.4 + 0.8. Thus, in children of group I, compared with group II, urodynamics in the newly formed pyeloureteral anastomosis recovered significantly earlier (p<0.05). This ultimately led to a reduction in the length of stay of children in the hospital. Thus, the average length of stay in the hospital in patients of group I was 9.2+0.7 days. The same indicator in group II was 14.34+0.9. Significance of differences between groups was p<0.05.

A master class was held and a lecture was given on modern methods of examination and treatment. A master class on the modern minimally invasive treatment technique “Gel plastic surgery of the anal canal” was held by 11 patients diagnosed with postoperative anal incontinence in children.

In 2022, an agreement was signed with the Department of Pediatric Surgery of the Russian National Research Medical University named after Pirogov, the Ministry of Health of the Russian Federation to organize an advanced training course in the specialty “Pediatric Endosurgery”. In accordance with this agreement, on February 13-17, Namir Adanovich, Associate Professor of the Department of Pediatric Surgery, Al-Mashat Russian National Research Medical University, visited the institute and the department. They gave lectures to undergraduate and graduate students and young professionals on the topic “Introduction to laparoscopy”.
From April 25 to April 29, 2022, a 36-hour advanced training course on the topic “endoscopic operations in pediatric surgery” was held. 15 cadets performed modern minimally invasive endosurgical operations with qualified doctors from Russia and exchanged experience. The results obtained will be introduced into the scientific and educational process of the department.
